I have reviewed the list of approved teaching composites, majors and minors. I choose one of the options listed below.

Option 1:

I understand that I must choose either an approved teaching major and an approved teaching minor, or an approved Composite Teaching Major, in order to meet requirements for Teacher Licensure. I choose the following Teaching Major or Composite Teaching Major as my primary area of teacher licensure________________________________. (This major is/is not (circle one) exactly the same as the major posted on my undergraduate degree transcript.

I have contacted the department of my teaching major to let them know I already have a degree and will be completing the courses required for the teaching major.
